Care Homes Team
Who are we?
Our team covers 5 care homes across Cambridge: Cherry Hinton Nursing Home, Home Close Care Home, Brook House Care Home, Heathlands House Care Home, and Cambridge Grove Care Home. Our model enables us to focus on implementing advance care planning, improving the quality and content of ReSPECT plans, and providing the care homes with dedicated clinical and pharmacy support each day between 8am and 6pm. Through this approach, we aim to reduce hospital admissions, GP callouts and the need for Out of Hours services. We also work closely with the CUH Care Home Geriatrician Team, allowing us to offer residents support and input from the community-based geriatricians.
Our Core Services
Weekly Ward Rounds
Each of our Care Homes has a weekly ward round. During this, our Paramedic or Pharmacy team can discuss and assess any acute or long-term conditions and discuss long term planning through the use of Advanced Care Plans and ReSPECT forms.
Monthly Multi-disciplinary team meetings (MDTs)
Our team, the Care Home, and the GP gather once a month to discuss a selection of patients, mostly focusing on long-term conditions, reviewing medications, and general stability.
Day to day support
Monday-Friday, 8-5, our email inbox is open for urgent or routine queries, medical, pharmacy related, or administrative from the Care Homes. During these hours, we always have a team member watching the inbox and triaging the queries to the most appropriate member of the team. Our inbox is also open out of hours and during weekends, but the queries will be picked up on the next working day.

Registrations
Registering new care home residents with their local GP service
Discharge Review
We will go over a care home residents discharge paperwork from hospitals and organise any medication setup or hospital-requested health reviews and care follow-up
Care Home Healthcare
Care homes contact us with their concerns for patients general health, particularly for their housebound residents. We also support GPs with Long Term Condition Reviews
Medication Optimisation
Whether it be a Medication Review, or an update from other local healthcare services, we will ensure residents medications are up-to-date and appropriate for their needs

As we are a team dedicated to serving local care homes, contact is organised by care home staff, though we can also be reached via the GP surgeries if needed.
