The Alzheimer's /Dementia Support Team operate out of Cherry Hinton & Queen Edith Medical Practices.
They offer face to face advice and support to families living with dementia.
Care Coordinators provide support and signposting for patients needing carers support, setting up additional help at home, home adaptations, health technologies (e.g falls alarms), and referrals to various community schemes (weight management, diabetes management etc).
Our In-House First-Contact Physiotherapy Team hold clinics in Cherry Hinton Medical Practice, Cornford House Surgery, Petersfield Medical Practice, Queen Edith Medical Practice & Woodlands Surgery, as well as providing a telephone consultation service for those unable to make it to their face to face clinics.
The Health & Wellbeing Team are tasked with providing support to people looking to improve their lifestyle & self-management strategies.
We support our patients using a non-judgemental and person-centred approach to build the confidence, knowledge and skills to make positive changes for themselves in all areas of health and wellbeing.
Social Prescribers support patients with non-medical issues. Empower them to take an active role in their wellbeing by signposting to appropriate organisations.
Our personalised care team is providing the Move with Us exercise class for our patients with Dementia and Parkinson's. We offer an hour of seated exercise to help people get moving again. These patients loved ones and carers are also encouraged to come along to socialise and discuss any worries or problems with the team.
