We support our patients using a non-judgemental and person-centred approach to build the confidence, knowledge and skills to make positive changes for themselves in all areas of health and wellbeing.
We are available to see patients who are aged 18 or over and are struggling with long term conditions and would like support in improving their ability to self-manage their health.
People who are simply looking to improve their physical health and wellbeing, be it through diet, physical activity or weight management, can be connected to local health improvement initiatives
You would need to be referred by a GP or other clinician. We would then contact you shortly after.
Most appointments are held by telephone or Zoom. However, face to face consultations can be requested and provided, with the location varying by availability.
